All players & parents are required to review and agree to the following before participating.

Medical Release Form– A sports physical designating your player is clear to play is required. Bring our Medical Release Form to your provider to fill out *OR* bring us a copy of the sports physical form you got from them. It must be signed and dated by your provider.
