Who's Ready for Football? 

All players & parents are required to review and agree to the following before participating.

Click on each item to review; you will agree to the contents of each form during registration, no need to print & fill out:

  • Medical Release Form– A sports physical designating your player is clear to play is required. Bring our Medical Release Form to your provider to fill out *OR* bring us a copy of the sports physical form you got from them. It must be signed and dated by your provider.
  • Copy of the player’s Birth Certificate for new players
  • Liability Waiver
  • Code of Conduct Agreement

Summertime heat can cause cramps, heat exhaustion, and heatstroke: all signs of dhydration caused by excessive sweating.
As with all injuries and illnesses, prevention is the key. When your children are out in the heat, make sure that they have plenty to drink. Water is the best liquid to give.
